The personal data that Care Clinic processes are the following: first and last name, gender, date of birth, place of birth, address, telephone number, email address, BSN, health, including the name and address of the medical specialist with whom you are being treated.

Purpose of processing personal data

The processing of personal data takes place under the responsibility of Care Clinic for the following purposes: to fulfill the treatment agreement with you, to provide services to you, to inform you about changes to our services and products, to deliver goods. and services at your place, sending and handling invoices and payments, sending newsletters and / or advertising and / or in the event that Care Clinic has a legal obligation to process and store your data.

Sharing personal data with third parties

Care Clinic will only share your data with third parties after permission has been obtained from you if this is necessary for the execution of the treatment agreement concluded with you and / or to comply with a legal obligation. We have entered into a processor agreement with the relevant third parties to ensure the same level of confidentiality and security. Sharing your data with third parties takes place via Medicore or via an email box secured by Zivver.

Access to personal data

You can request Care Clinic to view, change or delete your data, which request Care Clinic will meet. You also have the right to withdraw your consent to the data processing or to object to the processing of your personal data by Care Clinic and you have the right to data portability. This means that you can submit a request to send the personal data that Care Clinic has about you in a computer file to you or another organization mentioned by you, which request can be sent to afschriftdossier@dianagabriels.nl.
